  • Medical Information Form
  • Student's Immunization Record
  • A copy of the student's birth certificate or passport
  • 2x(2") student photos (taken within the last six months)
  • 2x(1") parent photo (taken within the last six months)
  • A minimum of the most recent 2-3 years of school reports
  • A copy of parent's passports and visas (foreign citizen)
  • A copy of parent's work permit (foreign citizen)
  • Application fee 5,000 Baht
